ABOUT THE HEALTH REGISTRY
The Chemical registry for this page was created so we can show Congresspeople, Senators, and other top officials, the necessary statistics they need that include a list of the side effects that our members are suffering from after being exposed to toxic chemicals in our military base ground water systems, from being subjected to burn pit contaminates, or from toxic incinerator emissions. We are doing this registry to show a true representation of the numbers of veterans and families who have been affected so that we will be able to compare our results with the various environmental studies that have already been conducted. We will maintain your privacy unless of course we are subpoenaed by a government agency, which may include a Congressional or Senatorial member. Please fill this survey out (1 survey for each member of your family that has been possibly affected). This survey will not only assist us in advocating for you with your state representatives but it will also help us to pressure your representatives in Washington D.C. for the healthcare and benefits that our military members and families deserve for our primary and secondary illnesses related to toxic chemical exposure.
Thank you for your support!

Zone Map Of The NAF Atsugi Incinerator Contamination Zone ***(NAF Atsugi Members Only)***
This is the map to use for questions about what zone that you lived or worked in later in the registry survey. Each number 1,2,3 & 4 represent a separate zone with the number 1 zone being the worst zone and closest to the incinerator. The locations typed in red will help you find a familiar reference point on the map.
